I've been on it for months and have never had ANY diarrhea. As long as you keep your fat lower than 15 grams per meal, you're fine. All bad effects from alli are totally based on YOUR actions. If you don't eat too much fat, you don't get any bad effects. So if you plan to eat Mexican food or cheesecake or something else that's really high in fat, don't take Alli for that meal.
